In February of 2007, the GlaxoSmithKline drug Alli became the first weight loss medication approved for over the counter sale in the United States. The active ingredient in Alli is orlistat, a generic name for the compound tetrahydrolipstatin. It has been available for about fifteen years as a prescription drug, currently being marketed by Roche under the brand name Xenical.

Alli is a smaller dose of what is prescribed in Xenical. Regardless of the name, the core ingredient in Alli has been available as a weight loss drug for nearly fifteen years. For that reason it has been something of a surprise that the FDA in August of 2009 declared that it was undertaking a safety review of Alli and the other drugs available with orlistat as the active ingredient.

Alli works to suppress the absorption of fat by the body. In doing so, the medication lowers the calories taken in by the gastrointestinal system. The unabsorbed fat is excreted naturally by the body. The medication has proven to be a reliable, if modest contributor to weight loss programs undertaken by obese patients.

Alli and its sister medications have also developed a potential association with acute liver damage as a side effect. The reason for the FDA review is that thirty cases of acute liver problems have been reported for patients who were taking Alli or another orlistat drug. All were hospitalized, and six experienced complete liver failure. Because of the intensity of the side effects the FDA has chosen to take preliminary action with relative speed.
